Admissions
Admission Process
To ensure a successful admissions process for our residents.
We do the following:

Needs Assessment and Individualized Care Plans
We conduct thorough needs assessments for potential residents to understand their specific requirements and challenges. Develop individualized care plans tailored to each person's needs, ensuring that they receive the appropriate level of support and care.

Collaboration with Referral
Partners
We establish partnerships with hospitals, community mental health centers, veteran support organizations, and senior care facilities to receive referrals. Maintain open communication channels with these partners to streamline the admissions process.

Application and Intake Process
We create a clear and straightforward application process, either through online forms or physical documents. Ensure that the application collects necessary information, such as medical history, support requirements, and personal preferences.

Admissions Committee
Our admissions committee comprising qualified professionals, including social workers, mental health experts, and veteran representatives. This committee can review applications and assess each applicant's suitability for the group home.

Transition Planning
Develop a comprehensive transition plan for new residents to ease their move into our residential care home. This includes communication with their previous support team and ensuring a smooth transfer of care.

By implementing these admissions ideas, our foundation can ensure a well-structured and compassionate process that prioritizes the needs of individuals with mental health challenges, intellectual disabilities, veterans, and aging adults, while fostering an inclusive and supportive community.
